  • IMMUNIZATION GUIDELINES FOR SCHOOL

    State of Texas Requirements for School Entry
    Hepatitis B: 3 doses for all grades

    DTP/DTaP
    Pre-Kinder: 4 or more doses
    Kinder - 6th grade: 4 or more doses including the 4 year-old booster
    7th grade: 3 or more doses including the 4 year-old booster
    AND
    Tdap/Td booster in last 5 years
    8th - 12th grade: 3 or more doses including the 4 year-old booster 
    AND
    Tdap/Td booster in last 10 years

    Polio

    Pre-Kinder: 3 or more doses
    Kinder - 12th grade: 3 or more doses including the 4 year-old booster

    MMR

    Pre-Kinder: 1 dose
    Kinder - 6th grade: 2 doses
    5th - 12th grade: 2 doses (or 1 dose measles and 1 dose MMR)

    Varicella (chicken pox)

    Pre-Kinder: 1 dose
    Kinder - 12th grade: 2 doses

    Hepatitis A

    Pre-Kinder - 6th grade: 2 doses
    7th - 12th grade: not required, but recommended

    Meningococcal (meningitis)

    Pre-Kinder - 6th: not required
    7th - 12th grade: 1 dose
    Second dose may be required to enter college
    HIB: required up to age 5 only (Pre-Kinder)
    PCV: required up to age 5 only (Pre-Kinder)

    Immunizations must be up-to-date before school starts August 24, 2015!

    SENIORS: MENINGITIS VACCINE REQUIRED FOR COLLEGE ENTRANCE!

    Effective January 1, 2012, Texas Senate Bill 1107 requires all students (under age 30) entering an institution of higher education (public and private) to either receive a vaccination against bacterial meningitis or meet certain criteria for declining such a vaccination before the first day of the semester.

    The vaccination must be received at least 10 days prior to the first day of the semester.
    Vaccinations older than 5 years will require a booster.
