Parental Consent (SB12 FAQs) for Health-Related Services
During the 89th Texas Legislative Session, lawmakers passed Senate Bill 12, which requires schools to get annual parental consent before providing routine physical or mental health services. This includes things like first aid, short-term counseling, or vision and hearing screenings.
To complete the consent form:
A copy of the consent has been sent to your email
Sign a copy of the consent form with your school nurse at “Meet the Teacher”
Visit your child’s campus to sign a copy
Indicate whether or not you consent, sign and return.

Why am I receiving this consent form?

What are “routine” school-based health services?
Services like first aid, over-the-counter medication per policy, vision/hearing screenings, short-term counseling, and emotional support.
What services are not included?
Diagnoses, prescription medication administration without authorization, or ongoing therapy without separate consent.
Who provides these services?
Licensed school nurses, athletic trainers, counselors, and other certified health professionals employed by PSJA ISD.
How do I give or deny consent?
Complete the attached form and return to your school nurse.
If I deny consent, what happens in an emergency?
In a serious emergency, staff will still act to protect your child’s safety and will contact you immediately.
Can I revoke my consent or denial later?
Yes. You can revoke or provide consent at any time in writing to your campus office.
Will my child’s health information be kept private?
Yes. All health records are protected under state and federal privacy laws.
