Pre-Participation Physical
All athletes, including cheerleaders, are required to get a physical for each school year.
All students will need to submit a new UIL Medical History Form and complete all online forms. Students with a history of certain conditions may need to get additional medical clearance.
Print out the form below and complete the medical history on the front page; the back page is for the doctor. After completing the physical, turn in to the athletic trainer for your campus.

Online Forms
Each athlete and cheerleader is required to have digital forms completed each school year. These are completed online at the link below. Both the parent/guardian and the student must sign these forms.
